Greetings, fCC Community!

I would appreciate feedback on my second project - https://codepen.io/terrencemm2/full/KxdaBy/

I spent about 8 total hours on this project attempting to fine-tune the mobile view, playing with submitting to Google Sheets (results can be found here -
https://docs.google.com/spreadsheets/d/1PYDprtiullQQk2o6AzlUZC8JWg2fejmLIu0cXG0nvbw/edit?usp=sharing), using Notify,js, and working with both flexbox and grid formatting. I will admit that I did have to look at the example once or twice to understand how to arrange the sections.

Good job, I like your footer a lot! I would,

change mouse cursor when it hovers on the submit button.
You might have intended it, but I would just grow age input box to max on mobile as well.

Check your labels. Not all of them have the for attribute.

Thank you, @sorinr. I was using some templates in order to quickly get the Google Sheets functionality to work.
