Tried to go for a clean minimalistic look, it’s a form after all.

Looks pretty good to me. Very clean and easy to read.
Only thing I noticed, and it might be nit picky, is when the dropdown is opened there is just a little bit of cut off text from the bottom of the check boxes and radio buttons that looked a little off to me. I don’t know if that’s anything to consider.
