Hey good try! I like your DOB placeholder. I would
change strategy and align labels to the left side and input boxes to the right. This will naturally solve lots of alignment issues.
Increase size of your input box for contact. Your placeholder is being cut due to it.
Try using different colors for your headers and intro text so they contrast more with your background for readability.
