Help! - Can't work out why User Stories 4 and 5 are not working on "Build a Survey Form"

Hi, can anyone help - I can’t work out what the mistake is.

I’m going the the list of errors after testing my survey page and I can’t figure out why the test has an issue with;

  • User Story #4: Inside the form element, I am required to enter my name in a field with id=“name”.
  • User Story #5: Inside the form element, I am required to enter an email in a field with id=“email”.

I’ve got these id’s in the HTML, have I missed something???
