Looks real good - you went above and beyond and it shows.
That said, if I wanted to be picky…
There’s two typos on the first page - Please fil up your infomration below - fill and information
If there’s a label for an input, does the input need to have the exact same placeholder text?
The email field doesn’t require a valid domain. (asdf@asdf works)
The previous button doesn’t need to display on the first page, because there is nowhere to go back to.
On the fourth page - the questions feels smashed together.
Another typo there (Proformance)
Another typo on the submit button.
When you shrink your browser, things feel a little cluttered and could be better.
This is me being about as picky as I can, mainly a few minor typos.
