What do you mean by arrange?
If you need help styling the form with CSS, here is an edited version of your pen.
I just centralized the form and added some margins/padding. And I edited the markup a little, like wrapping the text in <p> tags instead of just using <br> tags.
I hope that helps, tho I wasn’t sure what exactly you needed help with.
Also, where it says
“Are you satisfied with the performance of the team?”
Your input tags have names and values that do not correspond to the text.
