I just completed my Survey Page and would look some feedback on it.
I found styling the labels for the inputs in a way similar to the example really annoying so I fear my solution might be a bit hacky, so any input on that is greatly appreciated!

To be honest it’s really ugly.
The animation is pretty clean though.
Here are some good examples of REALLY pretty forms.
Study them, analyze them, figure out why theirs are pretty and yours is ugly 
You need to tidy this up by focusing more on the styling to make it look much more presentable. Otherwise the end product although passing the test runs, will not be acceptable or fit for purpose. 
Well i feel as if its mostly color scheme.
From there try and change up the fonts and text size and center everything.
