Nice and clean! I would,
Align profession select field with above input boxes.
Very likely radio box is located bit too high. Try to even out spaces like other radio boxes.
Align your second select input box with its corresponding question.
Reduce spaces between checkboxes.
Change cursor and color of button when mouse hovers on it.
