Your design looks good, I would suggest maybe adding some margin in between each of the input fields and maybe adding some padding to the inside of the input boxes as at the minute the form seems a little cramped and squeezed in.
Thanks for the suggestion Pezz, the changes are live now. Only one problem that still persists is the look of the page in editor view. Do you have any idea of correcting it.
